In order to support students as they move through Learning Communities towards graduation, we need to create a Transitions Manual for students who receive special services with grade-level transitions and post-secondary transitions. In this manual, we would establish clear expectations for the Transition process, support schools in establishing connecting skills between transitional grades (elementary to middle, middle to high), and build parent engagement around transitional process to ensure that all necessary transitions happen in a timely and efficient manner.
